Los Angeles Hospital Notary

1. Scheduling and coordinating

•    Call or Text: Contact us with
the hospital name and room number.
•    Confirm the Document: Let us know what you need signed ( e.g., Power of Attorney).
•    Coordinate with Nursing: It is helpful to let the nursing station know a notary is coming so they can pause non-urgent care during the signing
.

2. Ensure notarization can be completed before our arrival:

•    Awareness: The patient must be alert, coherent, and aware of what they are signing. If they are heavily medicated or unresponsive, the notary cannot legally proceed.
•    Identification: The patient must have a valid, unexpired government-issued photo ID. If they do not have one, please let us know immediately so we can discuss "Credible Witness" options.•    Document Readiness: Have all documents printed and ready. Do not sign them before the notary arrives.
•    Witnesses: Some documents (like Wills or Advance Directives) require additional witnesses. Check your document's instructions to see if you need to provide these.
